Please select the type of policy you wish to claim on:

When making a claim we will need you to provide the following information so that we can assess your claim as quickly as possible:

  • Details of your illness or condition. This includes dates of your first symptoms and the first day you were absent from work.
  • Your occupation details, including the name and address of your employer (if applicable).

We will also need to complete a financial assessment. To do this, we review your personal taxable income in the 12 months immediately before your illness or injury and require the following:

If you are employed If you are self-employed If you are a company director
  • Your last month's payslip or equivalent
  • Your last P60
  • Your last HMRC Self-Assessment tax return
  • A full copy of your latest certified accounts
  • Your last 3 months' payslip or equivalent
  • Your last P60
  • Your last HMRC Self-Assessment tax return
  • A full copy of your latest certified company accounts

 

Please also tell us about any other income that you may be receiving when your waiting period finishes.

This includes:

  • Payments from your employer e.g. Company Sick Pay (but not Statutory Sick Pay)
  • Any continuing income or dividends from your business
  • Pension payments (unless you received these before your policy started)
  • Payments from similar insurance benefits.

However you decide to claim, we will ask you for your contact details, details of the life covered and the type of claim you wish to register, either a life or terminal illness claim.

However you decide to claim, please have the name of the policyholder, the policy number and basic information about the reason for claiming close to hand.

Step 1:

Pay for your treatment and keep your receipt(s).

Step 3:

The money will be paid directly into your bank account (less any excess), usually within three days of the claim being assessed.

Step 2:

Login to the claim portal below with your reference number to hand and simply attach a picture or scan of your receipt(s). All claims should be submitted within three months of the treatment date.

To save on postage you may wish to hold on to receipts that are equal to or below the excess amount on optical and dental. You can send these with your next claim. All claims should be submitted within three months of the treatment date.
